1. Information we collect on this website
This website collects the information you choose to submit through our contact form: your first and last name, phone number, email address, the state you are in, the type of insurance you are interested in, your preferred contact method, and the content of your message. If you check the SMS consent box, we also record that you gave consent, together with the date and time and the version of the consent text you were shown.
Our public contact form is not intended for sensitive information. Please do not submit Social Security numbers, banking or payment details, medical records, prescription information, test results, or copies of identification documents through this form. If any of that information becomes necessary to complete an application, we will request it through an appropriate, separate process.
Like most websites, our hosting provider may automatically log technical information such as IP address, browser type and the pages requested. We use this for security and for keeping the site working.
